Do NSAIDs prevent bone healing?
To give [NSAIDs] or not to give [NSAIDs]; that is the question!

Pain related to orthopedic surgery, dental extractions, or any injury that involves connective tissue is caused in large part by inflammation which occurs because of prostaglandin release into the surrounding tissues.  Non-steroidal Anti-inflammatory Drugs (NSAIDs) block these prostaglandins and therefore should be a mainstay of pain treatment alone or perhaps kevin-ware-injurycombined with other agents such as opioids as long as there are no medical contraindications.  Or should they?  Certainly some patients could benefit from the combined use of NSAIDs plus opioids – this should presumably have been true for a case such as the well-publicized basketball injury endured last year by Louisville athlete Keven Ware But, there has been much unsubstantiated controversy over whether or not NSAIDs are beneficial or detrimental in cases involving bone healing.

Before delving into the facts regarding NSAID use post-orthopedic surgery, other very important issues lay the foundation on which the NSAID arguments are built.

According to a DEA Resource for Parents updated in 2012 Prescription for Disaster – How Teens Abuse Medicine, “Enough prescription painkillers were prescribed in 2010 to medicate every American adult around-the-clock for a month. Although most of these pills were prescribed for medical purposes, many ended up in the hands of people who misused or abused them.” (CDC Vital Signs, Nov. 2011) We know that 70% of youth get their prescription drugs from family and friends, yet fewer parents report safeguarding prescription medications. “Anyone can access prescription medicines in the medicine cabinet” went from 50% in 2010 to 64% in 2011, meaning medications are available to anyone in their homes.

Aside from these facts, this 2014 article in The New Republic: Drug Dealers Aren’t to Blame for the Heroin Boom. Doctors Are”, Graeme Wood criticizes his dentist for prescribing hydrocodone to relieve pain after his wisdom tooth extraction.

Adding insult to injury, some believe that Medicare’s patient satisfaction surveys may significantly impact opioid abuse.  I for one was mortified last year when asked to lecture to a large healthcare system in the Northeast.  While teaching on general concepts of acute and chronic pain management, I learned that patients frequently were admitted through the emergency department for 2-3 days at a time pretty much to receive intravenous hydromorphone (aka Dilaudid) for unidentified pain. And yes, they got the drug even with a clear history of substance abuse. Why?  Because the reimbursement model is based on their pain scores – and that may have been more important to the institution than doing the right thing medically.  But what the heck, right – it was only a few days of hydromorphone; no harm, no foul, the hospital system looks good, and the reimbursement is elevated.

There is a controversy on whether or not to treat with NSAIDS after orthopedic surgery, as various animal studies have found NSAIDs to delay bone healing. However this polemic stirs up significant angst and sometimes indifference among the surgeons who spend limited time addressing pain issues post-operatively unless there are pain-related surgical complications such as infection.

NSAIDs delay bone healing through their inhibition of prostaglandins.  Cyclooxyygenase (COX) converts arachidonic acid into prostaglandin G2, and then prostaglandin H2. Prostaglandin H2 is the precursor to active prostaglandins and thromboxane. COX1 is expressed in most cells and is involved in physiological processes such as reducing acid secretion in the gastrointestinal tract and regulating blood flow in the kidneys. COX2 activation is results in pain, inflammation and fever.  Selective blockade of COX2 therefore plays a critical role in reducing or preventing the inflammatory process following orthopedic surgery.  But, many studies have also found that COX1, and especially COX2, also play an essential role in bone formation, bone union, and resorption following a fracture.

I was asked to address some seemingly very simple questions.  I quickly found out that they weren’t so simple after all and the absence of evidence was astonishing and perhaps even unjustifiable.

Initial queries included:

  1. For what period of time after surgery do NSAIDs delay bone healing?
  2. After discontinuation of the NSAID, does time eventually make up for the delayed bone healing, and if so, are there adverse consequences?
  3. Are the diminished bone-fusion effects (if valid) reversible without adverse consequences once the NSAID is discontinued?

After an extensive literature search, I was unable to find compelling evidence to unequivocally answer these dilemmas, but new questions surfaced.

Does the avoidance of NSAIDs have potential for elevating intolerable pain that could otherwise be kept at bay?  And, could this uncontrolled pain following a bone fracture or orthopedic surgery lead to complications, such as the development of complex regional pain syndrome (CRPS), a chronic pain disorder that is extremely difficult to treat?

Complex regional pain syndrome is a chronic systemic disorder characterized by pain in combination with sensory, autonomic, trophic, and motor abnormalities. The exact pathophysiology of this disease state is not understood and therefore treatment of this syndrome is difficult.  Patients with CRPS have chronic pain which in many cases can lead to disability. Many studies have found fractures and post-operative pain as risk factors for the development of CRPS. A population based study by Sandroni et al, found that previous fractures was the most common trigger for CRPS in 46% (34 out of 74 cases) of identified patients with CRPS. This study estimated an incidence rate of CRPS as 5.46/100 000/year. De Mos et al, found the overall incidence rate of CRPS was 26.2 per 100,000 person years and that a previous fracture was the most common precipitating event occurring in 44% of identified patients. Furthermore, a study published last year by Moseley, et al found a correlation between the level of pain post-fracture and the development of CRPS. They concluded that having a pain score of  >5 on a scale of 1-10, as early as one week after wrist fracture puts the patient at a higher risk for development of CRPS. While the data is lacking on which fractures or surgeries most commonly increase the risk for developing CRPS, the most common ones seen through a literature search included fractures of the distal radius and post total knee arthroplasty, after fasciectomy for Dupuytren contracture, and after carpal tunnel surgery.

Through my extensive literature research I found that the effects of NSAIDS on bone healing were undoubtedly inconclusive.  There have been no randomized controlled trials on the effects of NSAIDs on post-operative healing in humans. Every randomized controlled trial to date is an animal study which includes rats, mice, rabbits, goats, and even one trial on dogs. Animal data cannot necessarily be extrapolated to humans. The majority of trials reviewed studied fracture healing in the short-term. Many trials looked at fracture healing as early as 21 days post-surgery and at latest, 12 weeks post-surgery. Even though the healing time of a fracture in animals may differ compared to humans, normally it takes months for a fracture to completely heal. Therefore the short-term effect of NSAIDs on overall fracture healing in the long-term is unclear even in animal studies. While NSAIDs may or may not delay healing of fractures, if the fracture completely heals within a reasonable time period, does it matter how long it takes if there is a significantly lower risk of developing complications (i.e. CRPS, depression, anxiety, opioid-dependence) post-surgery?  In my mind this is quite puzzling.

Orthopedists today generally hesitate to treat post-operative pain with the COX2 selective NSAID celecoxib (Celebrex). This is likely due to the retraction of several studies by Dr. Scott Reuben supporting the use of COX2 selective NSAIDs in the management of post-operative pain. Dr. Reuben, the former Chief of acute pain services at Baystate Medical Center in Springfield, MA, published various randomized controlled trials from 1996 to 2008 on the use of  COX-2 specific drugs celecoxib, rofecoxib, and valdecoxib either pre-operatively, intra-operatively, or post-operatively. Due to his positive findings, many physicians began routinely prescribing COX2 selective NSAIDs for the management of post-operative pain.  However in 2010, it was found that Dr. Reuben fabricated the data in at least 21 of his articles. In essence, according to Scientific American, over the course of 12 years, “anesthesiologist Scott Reuben revolutionized the way physicians provide[d] pain relief to patients undergoing orthopedic surgery for everything from torn ligaments to worn-out hips. Now, the profession is in shambles after an investigation revealed that at least 21 of Reuben’s papers were pure fiction, and that the pain drugs he touted in them may have slowed postoperative healing.” After this, orthopedists began to question the validity behind prescribing NSAIDs for post-operative pain.

Interestingly enough, I found many animal studies which compared the effects of COX2 selective NSAIDs with non-selective NSAIDs in delaying fracture healing. While some studies concluded COX2 selective NSAIDs delayed fracture healing more than their non-selective NSAID counterparts (such as ibuprofen or naproxen), many other studies found the opposite to be true. COX2 selective NSAIDs include celecoxib (Celebrex), rofecoxib (Vioxx, taken off market), and valdecoxib (Bextra, taken off market).  Etodolac (Lodine) too is very COX2 selective however has never been marketed as such.   Click here to see Cox-2 specificity by drug.

Conclusions of my literature review matched that of many other review articles and expert opinion articles published on the topic. These articles emphasized the contradictory nature of the topic and the need for large randomized clinical trials in humans.  A summary of all the literature reviewed appears below for your reference.  Perioperative NSAID use for orthopedic surgery is surely a ripe area for study!

  1. I recently had navicular bone fusion surgery a month ago. I take indomethacin for shoulder and wrist and neck pain. Dr said not to take at all, said take Tylenol arthritis for pain. I have been to the ER twice. once because of a gout problem on other foot and other time because I was in such pain that I had a hard time just getting up and was miserable. Taking indomethacin for2 days got me out of pain both times and then I stopped taking it. Where does quality of life come into pain. I don’t want to delay my foot from healing. Not sure what to do!!

  2. Pretty simple concept:
    If we haven’t found a clear signal that NSAIDs delay fracture healing, they don’t delay fracture healing.
    To every question, there are an infinite number of wrong answers.
    We do know that administration of opioids/intoxicants past a few days leads to problems; that signal is clear.
    Use NSAIDs.

I have had over a dozen surgeries between my back and neck and just recently had a total knee replacement and was taking Tylenol and oxycodone after surgery but I’m over 3 weeks out and ran out of oxy and was trying to baby them but the problem I’m concerned about is I’ve had a pain level that sometimes goes up to a 7 and knowing I’m not supposed to take too many Tylenol at at a dosing period i just recently started taking 4 every 4hrs because of extreme pain sometimes interfering with sleep and that is all I had except what I saved for Pt which was only 2 per visit till my insurance changes. I started feeling sick and had diarrhea and felt I should stop with Tylenol get a new script and use heat. But I did find aroma therapy candles seem to help calm the nerves and relax and ease pain.

    A limited study, to be sure. But as a pediatrician, not an orthopedist, I am looking for answers to simple questions regarding pain relief in non-surgical fractures. I do not believe I should be telling my orthopedic colleagues how to manage their surgical patients.

    In the modern world, for simple fractures managed by primary care, I manage pain with ibuprofen and / or acetaminophen, rarely more than a week Other pain modifying modalities, such as distraction and up-beat music, are preferable to opioids and narcotics. Life has pain. Fractures heal.

    I worked 3 years in a Third World setting. Ibuprofen and acetaminophen were all we had. I was not dissatisfied with their effectiveness. And when I sustained my own fracture their, it is what I used on myself.

  5. I find it refreshing to find someone admitting when there is no unarguable answer. I became a Physicians Assistant in 1980, had my own first significant orthopedic surgery (total knee replacement) two weeks ago, and notice from then to now it was and is possible to find those who say NSAIDs harm, don’t affect, or help healing post op. Since there is no consensus, I prefer to not wishfully believe there is. I’m taking naproxyn 440 mg twice a day. It’s a dose I have taken occasionally in the past for sprains and strains, with no ill impacts other than easy bruising. Is it reducing my swelling and pain? Maybe. Swelling and pain are going down. Over the years since 2002 I have had a hip replacement (AVN after hit by a car) several years later the other hip was replaced due to OA, 2012 was my first knee replacement and 2014 my second (both done at NE Baptist hospital in Boston) Significant pain pre and post replacements. Relief was obtained with Vioxx,
    (until off the market) then ibuprofen, tylenol PM at night. Opioids prescribed post op but not used after 2-3 weeks home. I was never told not to take ansaids nor that my healing was less then what was expected. I returned to my job as homecare case manager after all surgeries within 12 weeks, lots of physical activity required. Always x strength tylenol suggested (never effective for me)
    ! month ago I underwent back surgery . Discharged on neurontin and roxicet and of course x strength tylenol. Pain has been hard to control (developed sciatic pain especially at night) Calls to MD resulted in increased dose of both neurontin and opioids. Also using ice and topical ointments with some short term decrease in pain. While using cane for balance I injured my rotator cuff L shouider. I was told by my surgeon absolutely no ansaids. Finding this incredibly difficult and started looking for articles and information on this as it has such an impact on pain management and recovery. Ambulation and sleep are
    difficult as are ability to stay positive. I found this article helpful and hope there is indeed more research going on. Thank you for this site Pam

    Re: NSAIDS in the peri-operative period, one must also consider the potential cardiovascular downside of stopping NSAIDs cold.

    1. I couldn’t agree more, especially when using non-selective NSAIDs for 3 months opr longer prior to the procedure where one is predisposed to cardiovascular risk. Thank you for the note!

  8. Fascinating article about using NSAIDs after surgery. Sadly the author was not able to find answers to the very questions I have – ie if NSAIdS are discontinued after surgery is the impact on bone healing reversible? In my case – I have been taking Mobic max dose for 20 years for joint issues. In January 2016 I suffered a spiral femur fracture and had surgery. Continued taking Mobic until 12 weeks after surgery! None of the orthopods mentioned the adverse effects of NSAIDs on bone healing! I discovered this from my own research after Xrays at 6 and
    12 weeks showed no sign of healing. I have now reluctantly weaned myself off Mobic. What I want to know is – and cannot discover after days trawling the internet – is the answers that Dr Shah could not discover in her article – is stopping NSAIDs 12 weeks after the fracture

  9. have severe osteoarthritis and needed an anterior cervical diskectomy fusion. I knew to stay off diclophenac 100 er prior to surgery, but was not informed that had to stay off it till fusion occurred (4-24 months). If I had been informed, I would have postponed or omitted the surgery. Now surgeon’s PA says I can take it after first four weeks, but can’t find any documentation that it would be ok after 4 weeks. My chronic pain and depression are worse. I don’t know what to do.

I share your frustration re no info available on stopping NSAIDs after surgery and timeframes re starting etc. There is loads of info re the adverse effects of NSAIDS on bone healing but thats it!   10. After having decompression, bone graft fusion surgery of my c 5-7 vertebrae – narcotics and opiods were prescribed. I never took anywhere near what was prescribed. I felt absolutely horrible on the cocktail so I went off all meds – unbeknownst to me I went into full blown withdrawal. It was absolutely horrible. Once off all drugs the pain and muscle aches (i am on a estrogen inhibitor due to breast cancer) were excrutiating. Tylenol did nothing. The only time I’m not in pain is when I am in a warm bath with Epsom Salts. The other day I was in such pain at work I took an Aleve even though I knew it was contraindicated for the bone graft healing. For the first time in months I was out of pain. Finally relief. I called my doctor and informed them of what I had done and they said Do Not Take Aleve – How about Tramodol, Hydrochodone, Soma,…….???? I guess I will spending the next months in my tub. Unfortunately I can’t bring my bath to work. I sit and cry at my desk. This constant pain is ruining my life. I have also tried walking – moderate exercise, meditation. minimal help.

    1. Show your doctor this blog!!! Perhaps She needs to review benefits versus risks, the supportive literature or lack thereof and make a decision after a complete review which I have laid out here. The final decision of course needs to come from your doctor.

  11. I have arthritis and find that the only thing that helps manage the swelling and pain are nsaids or a combination of both. It try 1 advil and 1 Tylenol when the pain is severe. I just had surgery and was taken off nsaids altogether and have found the pain meds do little to nothing for the throbbing and swelling. It’s very frustrating. Tylenol 3 works for about 3, 4 hrs, then the shooting pain and throbbing combine into a symphony of torturous pain. I want to follow drs orders but i also don’t want to be popping narcotics like Tylenol 3 or worse, every 2 hrs, there has to be a decent medium drs can figure out. If this goes on, by the time this is over I’ll be addicted to pain killers and I’ll have an even bigger problem in my hands. Please share opinions.

  13. Thank you for the insightful article. As a hospital RN of 33+ years and now as a recovering hip replacement pt. I am seeing 1st hand the need to move off of opioids to something with less side effects of constipation and drowsiness. The desire to continue the rehab of the leg is very important and to have some pain management that is less agressive as the opiates is what most are looking for. Control of how they feel and to get back to normal would be the goal, hoping not to slow the bone healing that the NSAIDS might do. To get a handle on how much and when nsaids are started post op would be a good study….. Thanks!

When it comes to people suffering from ongoing chronic pain for no good reason then take NSAIDS till their stomach bleeds causing yet another problem we must ask ourselves to what extent are we willing to go for pain relief. Withholding the correct medications for pain for whatever reason , fear, is ridiculous.
    THERE IS NO WITNESS SO DREADFUL ,NO ACCUSER SO TERRIBLE , AS THE CONSCIENCE THAT DWELLS IN THE HEART OF EVERY MAN .

  15. Thank you so much for this article. As I have written in the past, I can’t take opioids, and OTC NSAIDs at a low dose has always been recommended for my chronic pain over the years. My intolerance for medications extends to NSAIDs if the maximum recommended dose per day is met for even one day, so abusing them was never an issue — I just used them regularly for years. Recently severe GI problems were identified as a result of this. I am indeed fortunate that my pain management physician found a suitable alternative that I am able, so far, to tolerate.

    All of this points out the very sad fact that chronic pain is still an understudied, unrecognized health problem which results in lowering productivity at rates close to those of drug or alcohol abuse in America. Unfortunately, understanding how to effectively deal with all of this as both a health problem and a national economic problem is still clouded n mystery. There are solutions, and God bless all of you who seek them!

    Recently, a close friend endured a very complicated wisdom tooth extraction, made much worse due to the abscess of the gum and root entanglement with the jawbone. By the time the root cracked, broke off, and the protruding jawbone was filed down, she was warned to expect a great deal of pain. She received a mouth rinse, some Peninicillin VK, and 14 Motrin, 400 mg.

    While extremely sore and suffering quite a bit by that night, she improved until about three days later, or Thursday that week. Then she was unable to eat without intense pain that “felt like razor blades” and shot up to her ear. It seemed obvious to me the antibiotic was not working and now she had a large white ulcer on her tongue from where the jawbone was protruding. Since she had not improved by Saturday, she called the doctor on-call, as this was done at the place I used to work (the place that doesn’t give out ANY scheduled medications), the dental clinic had no on-call dentist.

    Upon hearing the doctor on-call was someone we had known for thirty years, we felt better about the situation. She changed the antibiotic to Clindamycin and offered some tramadol, to which my friend said no way. I suggested Tylenol 3, since NOTHING ELSE can be called in. This doctor actually called in 12 Tylenol 3s. It’s a sad, sad world when a person has to suffer to this degree for no reason.

    I don’t know how the Motrin effected her healing, but I know she suffered for nearly two weeks over something that should have been dealt with more mercifully.

    Thanks for this update. We need to be reminded that NSAIDs are not a “free pass.”

    Even back in the July 1998 issue of The American Journal of Medicine, NSAIDs risk awareness was evident. Yet, because death from NSAIDs is not considered a “character flaw” (i.e., addiction), and even perhaps a “nobler” way to die than from opioids, there is no public outcry – despite the fact that people continue to die by the thousands from NSAID related complications.

    This is sad.

    “Conservative calculations estimate that approximately 107,000 patients are hospitalized annually for nonsteroidal anti-inflammatory drug (NSAID)-related gastrointestinal (GI) complications and at least 16,500 NSAID-related deaths occur each year among arthritis patients alone. The figures of all NSAID users would be overwhelming, yet the scope of this problem is generally under-appreciated.”
