I’ve been on Beluca 150 mcg for almost two years (also: Cymbalta 90 mg qd, Lyrica 300 mg q. hs, Norco 7.5 mg tid, Tizanidine 2 mg prn, and a compounded cream). Last month my urine screen showed only a small amount of it in my system, and this month it showed none!! I have now been told I’ve violated my pain contract! I am an RN (currently working from home) and would NEVER do anything to jeopardize my Nursing license. Can you please help me understand why this could be happening? I appreciate any assistance you can provide. Thank you so very much. Julie

Julie, This is expected. The buprenorphine cut-offs are high, and often don’t detect buprenorphine unless patients are on doses consistent with SL dosage forms used of OUD. Also, you are sort of on the border for the cut-off on hydrocodone.
